Running orders tracker. Medications, IV and fluids, procedures and pending orders in one running log, so nothing sits forgotten in the queue.

Reassessment loop. Hourly vitals grid with patient response notes and an intervention log. The part that gets skipped when it's busy, made quick enough to actually do.

Disposition & handoff. End-of-shift patient status, report given, and outstanding tasks — so you leave clean and the next nurse isn't guessing.
